We are an out-of-network, cash-only practice. Full payment is required at the time of service and according to the agreed upon plan.
* Memberships are a 1-year contract.
* Members over the age of 18 years old: $99 per month
* Members under the age of 18 years old: $59 per month
* Students ages 18-24 with a valid school ID: $59 per month
* Membership and enrollment fees cannot be submitted to insurance for reimbursement or to be applied to your deductible.
* Membership plans do not include the cost of any medications, comprehensive labs, or referred Specialist's fees.
DPC Plan
A straightforward plan for individuals and families that covers essential health care throughout the year. Includes comprehensive primary care, annual and wellness visits, preventive and acute care, chronic disease management and more! Receive extended appointment times—so you receive unhurried, personalized care when you need it most.
Wellness Plan
A more extensive plan that provides full-spectrum men and women’s health services designed to support you through every stage of life. We focus on hormone evaluation and bioidentical hormone replacement therapy (HRT) and weight loss management to restore balance, ease symptoms, and help you feel your best.
Combined with nutrition, lifestyle, and functional medicine strategies, our approach supports lasting wellness, energy, and vitality.
